Have you recently been touched by the death of a loved one, or know someone who has?
At some point in our lives most of us will or have come across a situation where we or someone we love has been diagnosed with a serious illness, disability or terminal illness.
If it happened to you have you ever thought about where you would like to be cared for, what treatments you would and wouldn’t want, who would look after you or speak on your behalf if you weren’t able to?
These are difficult questions however it is important to consider them in advance to help family, friends and carers.

Advance care plan
Another useful booklet is for you to record and keep is the Advance Care Plan. It is not a legally binding document but more of a place to help guide you during your illness.
